The Five Towns Premier is committed to resuming visitation for residents, families and resident representatives while ensuring safety and adherence to infection prevention strategies to minimize any potential spread of infection. The facility has met all the criteria to allow visitation except for item number 4 in the policy which requires the absence of any new onset of Covid-19 test result among staff or residents. This is a Department of Health and CMS requirement and not the choice of the facility. Unfortunately this is not an issue for only this facility but an issue many facilities are facing. Please see this Dept. of Health memo dated September 17th for additional information. The facility will update this webpage once ALL the criteria are met to allow visitation as per the attached policy. If you have any questions of concerns please reach out to facility Administration.
Our residents receive a custom recovery plan – based on the initial evaluation through final discharge – designed to regain their independence when engaging in daily living activities. We offer physical, occupational and speech therapy on a daily basis, 7 days a week, allowing our residents to efficiently reach their rehabilitation goals.
The Premier Care team has been lauded for their person-centric approach to care, applying progressive therapy approaches in a distinctly warm, nurturing manner.
